In consideration of the opportunity to participate in a soccer league organized by Lake Town Athletic Club ("Organizer") at Montague Township Parks, during the summer of 2025, I agree as follows:

1. Acknowledgment of Risk
   I understand that soccer involves inherent risks, including injury, accidents, or death, resulting from actions, negligence, or inactions of my own, other participants, or third parties.
2. Release of Liability
   I, along with my heirs and assigns, release and waive any claims, actions, or liabilities against the Organizer, Montague Township, and all associated parties for any injuries, death, property damage, or other consequences related to participation in the soccer league or being on the premises, regardless of fault.
3. Indemnification
   I agree to indemnify and hold harmless the Organizer and Montague Township from any claims, demands, losses, or expenses (including legal fees) arising from injuries, losses, or damages during participation in the soccer league, whether caused by their own actions, others, or the conditions of the premises.
4. Assumption of Risk
   I voluntarily assume all risks associated with personal injury, death, or property damage related to participation in the soccer league. I affirm that I am physically fit to participate and understand the potential risks involved.
5. Compliance with Rules
   I agree to follow all rules and instructions set by the Organizer and the authorized personnel involved in the league. Failure to comply may result in removal from the property and removal from the league without a refund.

By signing, I confirm I am at least 18 years old, have read and understood this Agreement, and it involves my waiver and release of significant rights and my assumption of significant indemnification responsibilities in participating in this league, and I voluntarily accept its terms.
